Congratulations on making the decision to invest in your wellbeing! Complete the appointment request form and select a date and time for your free phone consultation. I will review your form and get back to you in 1-2 business days.
During your consultation, we will discuss your goals for therapy and what you are looking for in a therapist. We will then determine if we would be a good fit to work together. If we are, we will schedule your first session and get to work!

Individual Therapy
I accept most commercial Aetna and Horizon Blue Cross Blue Shield plans. I do not accept any other commercial plans or Medicare or Medicaid plans of any kind.
If you have out-of-network benefits, I can provide a special insurance receipt called a superbill that you can submit to your insurance company for possible out-of-network reimbursement for your session fees (see the next question).
Couples therapyI DO NOT accept insurance or provide superbills for couples therapy.
In order to use in-network or out-of-network insurance benefits, services provided must be “medically necessary” according to the insurance company’s definition. At a minimum, the following must exist and be documented:
An identified patient (must be an individual; cannot be a couple)
A diagnosis for the identified patient based on criteria outlined in the Diagnostic and Statistical Manual of Mental Disorders (DSM) (e.g., Generalized Anxiety Disorder, Major Depressive Disorder)
Impairments in the patient’s functioning as a result of said diagnosis
Interventions provided in sessions to alleviate symptoms and improve functioning specifically related to the diagnosis
At Evolution Counseling Suite:The couple (consisting of both individuals) is the identified client/patient AND the focus of the sessions (not either one of the partners individually). Couples therapy works best when the focus of the work is on the relationship.
Couples therapy is provided to improve the quality of the relationship and the bond between the couple as well as help them find clarity and work through issues in the relationship. It is NOT meant to diagnose or treat any mental illness. Neither partner will receive a diagnosis. If one or both members of the couple are experiencing issues with their individual mental health, it will be recommended that they attend individual therapy (with a different provider) in addition to couples therapy.
Feel free to reach out with any questions.

The best way to determine if you have out-of-network benefits and what you need to do to utilize them is to call your insurance company. Call the customer service/member services number on the back of your insurance card. Make sure you document the name of the representative you speak to, the date and time of the call, the call reference number, and the information the representative gives you.
Here is a list of helpful questions to ask when you call:
Do I have out-of-network benefits for outpatient psychotherapy/counseling?
Is there a deductible I need to meet before I can use my out of network benefits? If so, what is the deductible amount?
What percentage of the session fee will I be reimbursed?
What is the allowed amount per session?
Is there a limit on the number of sessions I can be reimbursed for? If so, over what time period (e.g., per year, per quarter)?
Is there a pre-authorization process I must complete before my first session?
How do I submit superbills?

During your first appointment, I will get to know you better. I will ask you a series of questions to get more information about your history, the concerns that led you to seek therapy, and your goals.
-
How long you need to attend therapy depends on your needs and goals. I will work with you to determine what your needs and goals are, and then help you learn skills and strategies to experience tangible and lasting improvement. My goal is to help you meet your goals as soon as possible. Some clients attend short-term therapy and “graduate” once they have met their goals. Other clients choose to make therapy a part of their ongoing self-care plan and attend therapy for an extended period of time.

As of January 1, 2022, health care providers and health care facilities are required to give a Good Faith Estimate of expected healthcare charges to every new and continuing client who is either uninsured or is not planning to submit a claim to their insurance for the healthcare services they seek.
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services, including psychotherapy services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
